Epilepsy Team Introduction

     
       The Pediatric Epilepsy Team at the Neuroscience Center specializes in the treatment of epilepsy in children. The incidence of childhood epilepsy in Taiwan ranges from 0.6% to 0.7%, with over 20% of cases being refractory to medication alone. These cases, often associated with brain abnormalities or evolving symptoms, may require epilepsy surgery performed by pediatric neurosurgeons.

       Before epilepsy surgery, a thorough understanding of the patient's medical history and identification of the underlying cause are essential. For patients with unclear etiology, preoperative assessments are conducted, including specialized brain imaging to observe abnormalities and nuclear medicine scans to assess glucose metabolism.

       The initial phase of treating patients involves understanding their epilepsy history, including video EEG monitoring, MRI&fMRI, SPECT/PETand neuropsychological evaluation. Subsequent evaluation phases (phase 2) may include invasive procedures such as stereoEEG and extraoperative strip/grid electrocorticography Epilepsy surgery requires collaboration between neurosurgery and neurology, with pediatric/adult epileptologists playing crucial roles. Additionally, rehabilitation assessments, specialized neuroimaging, and collaboration with anesthesia and ICU teams are essential components of the multidisciplinary approach to treating pediatric epilepsy.

       Epilepsy surgeries encompass various approaches, including palliative surgeries and resective surgeries. The goal of surgery is to eliminate epileptic manifestations even without medication. However, success rates vary, with only 70-80% of cases achieving seizure control. For cases with less distinct manifestations or when the epileptic focus cannot be clearly identified, palliative surgeries, vagus nerve stimulation, corpus callosotomy or deep brain stimulation may be considered to mitigate symptoms. In addition, ketogenic diet is one of important therapy for epilepsy patients other than many newer generation of antiseizure medication.
